How do I get explanation of benefits from VSP? ›

The University of California and VSP provide you with an affordable vision plan. For complete vision benefits information or to review the VSP Evidence of Coverage booklet, visit the UC benefits website at ucnet.universityofcalifornia.edu. To request a copy of the VSP Evidence of Coverage booklet, call VSP at 866.240.

Get Coverage on Your Own

Plus, there's no waiting period; you can start using your vision benefits the same day you enroll. Benefits you can expect with your VSP individual vision plan include: Low Out-Of-Pocket Costs—Save more than $300* a year on eye exams, glasses, and contacts.

You can use the plan at any Vision Service Plan (VSP) network provider nationwide, including most Walmart Vision Centers and Sam's Club Optical. Before you get service, make sure ​your provider​ is ​in the​ VSP network. Eye exams: $4 copay, once every calendar year.

You will receive one spectacle examination per Calendar Year, plus one of the following every two Calendar Years: (1) $200 toward the retail cost of eyeglass frames and full coverage of eyeglass lenses or (2) $300 toward contact lenses and contact lens fitting and evaluation fees.

EOBs are usually mailed once per month. Some plans give you the option of accessing your EOB online. Your EOB is a summary of the services and items you have received and how much you may owe for them.

Click on Shop Eyeconic from the left navigation, then the Check Eligibility button. If you are eligible, click Connect My Benefits and start shopping for frames and contacts online. If you have out-of-network benefits you can still purchase from Eyeconic.com and submit for out-of-network reimbursem*nt.

Most VSP members can use their vision benefits to buy eyewear online at Eyeconic—the in-network, VSP online store for glasses, contacts, and sunglasses. Plus, you'll save 20% off any out-of-pocket eyewear costs, just for being a VSP member.

Purchase the eyeglasses or contact lenses. Some popular places that accept VSP Insurance are Costco, Walmart, Sam's Club, and Pearle Vision.

Target and Walmart: If you'd rather not get a membership to Costco or Sam's Club, you can still get cheap eye exams while you shop at Target or Walmart. Eye exams at these major retailers also fall in the $50 to $100 range.

Discover all current eyewear offers and savings at vsp.com/offers. 20% savings on unlimited additional pairs of prescription or non-prescription glasses/sunglasses, including lens enhancements, from a VSP provider within 12 months of your last WellVision Exam.

Does VSP give you a card? ›

VSP does not generally give out ID Cards, but they are available online for you to print. You also have the ability to change your VSP account information.

How do I check my claim status? Once you've completed the out-of-network claim form, you can check your claim status on the Benefits History page on your account dashboard.

You will receive reimbursem*nt through your company's payroll process for covered services after you submit a claim online. How do I submit a claim for reimbursem*nt? You must be a registered user to access the claim form. Login to submit your claim form or click here to register.

It is often your social security number, but it may also be a unique ID number. You won't need a group number, group ID, or policy number to use your benefits. If you're unsure of what your member ID is, you can check with your benefits office or call us at 800.877. 7195.
